Weekly Van Sign-UP

Due to Texas law regarding child transportation safety and insurance liability, May First Baptist Church will no longer transport students who are younger than 9 years old, shorter than 4'9", or otherwise require a car seat or booster seat.

This form must be filled out weekly before 12:00 pm every Wednesday to ensure your student is included in the pick-up/drop-off schedule for that week. It will be available every Monday-Wednesday that the vans will run.

Spots on the vans will be reserved based on order of sign-up. In the event we have more students than seats on the van, we will notify you if we are unable to pick your students up.

NOTICE: IN ORDER TO RIDE THE VAN YOUR STUDENT(S) MUST ALREADY BE REGISTERED AS A MAY STUDENT OR MAY KID IN ACCORDANCE WITH THE APPROPRIATE FORMS ON
