This form should be filled out weekly before Noon on Wednesday to ensure your student is accounted for to be picked up and dropped off. It will be refreshed and open to be filled out Monday-Wednesday weekly.
The spots will be reserved based on order of sign-up. In the event we have more students than seats on the van, we will notify you if we are unable to pick your students up.
